Trip Application Form".
Patient full name (surname, last name), gender, birth of date,
occupation (job), citizenship, passport number, home address (street,
city, country), post code.
2, Please fill out the "Medical Trip Application Form", and then e-mail
back to us. If need tour or sightseeing in China, please tell us too
(tell us the cities name for you to visit, and in general, we will
arrange you tour before medical treatment), then we will quote you the
tour cost and make tour schedule for you. Maybe we will e-mail you again
for more disease or disorder information.
3, We make out "Medical Trip Advice" according to your "Medical Trip
Application Form", and e-mail you the "Medical Trip Advice", it shows
you that: trip advice, treatment advice (therapy, hospital, doctore
reference), estimated cost for trip and medical treatment, also tour
cost if necessary.
4, Basis above "Medical Trip Advice", you'd decide that which one is
your preference, and tell us your choice, your date to China for your treatment, then we will do make pre-arrangement for your
medical trip.
5, We will e-mail you the arrangement per your choice, and also
e-mail you the cost (China domestic trip cost with our service charge, treatment cost, tour cost if need) and
payment information.
6, You make sure the
medical treatment trip
arrangements in China. And make payment (marked in our "Medical Trip
Advice") to us: 1, trip cost (domestic in China, with our service
charge); 2,
deposit of medical treatment (usually deposit 20% of estimated medical
treatment cost, but not less than 3500CNY); 3, tour cost if you need.
7, After your payment reached us, then we will e-mail you the
"Trip Confirmation and Arrangement".
8, You, be ready to China, inquiry your local doctor for this trip..............
9, Before your travel, check with the airliner that you are allowed to travel.
And also inquiry your doctor for this travel, consider the impact (long
distance travel will have) on your health. .
